First Visit

Before you arrive at our office, please assist us by inputting your medical history using the Patient Login button above. It will help expedite the check-in process. You will need a login and password to complete this section. If you did not receive this information when you scheduled your appointment, please contact our office and we will be happy to provide it to you. If you are unable or prefer not to complete your registration using the Patient Login, your arrival time will be adjusted to allow you sufficient time to complete the registration process in our office.

PLEASE NOTE: All patients under the age of 18 years must be accompanied by a parent or legal guardian.

 

What to bring

Please bring your referral slip and a list of the medications you are currently taking. If you have dental insurance, bring your current insurance card, the subscriber's name, and date of birth. This will save time and allow us to help you in the processing of your claim.

 

Check in

If you completed your registration process through the patient login on this site, when you arrive at Endodontic Associates you will be given a tablet to review and sign your medical history and consent forms. If you did not complete the registration in the Patient Login area of the site, you will be given a tablet to complete the registration process. Please have available your medical history information and your medication list to assist you in the completion of this process.

 

Appointment

Most appointments at Endodontic Associates will consist of taking and reviewing x-rays (2D images) and 3D image scans, evaluation, diagnosis, and consultation with the doctor regarding treatment options. Depending on your appointment type, treatment may occur on the same day or a different day from your consultation. 

If your dentist has taken x-rays, please have them forwarded to our office (mail or email) as they may be helpful in our diagnosis. If additional films are necessary, they will be taken at our office.
